Please note that the documents you send will be responsibly disposed of by our company and will not be returned. Additionally, please be aware that there may be cases where we cannot respond to requests for disclosure, etc.

If the identity of the individual or their representative cannot be verified

If there are deficiencies in the prescribed application documents
When the subject of the request for disclosure, etc., does not correspond to 'disclosure-target personal information' (*)
